A: Thick flaring alar regions are difficult to manage

Thank you for the photos and question. Your photos show thick skin and tip without a lot of definition. Rhinoplasty primarily aims to alter the structural framework of the nose to achieve the desired physical changes. Thick skin obscures the best of structural work. While there are described techniques to thin the alar rim by thinning the skin, these techniques entail significant risks and yield limited results. I recommend you meet with a reputable and experienced rhinoplasty surgeon to discuss your desired endpoints and the limitations imposed by the unique characteristics of your nasal tissues.
